Leading from a Distance book possibly leading to surv injuries

Bought the 4 hour/60% auto chance Leading from a Distance book and having survivors returning with injuries from successful Very High autos. The missions are successful, so I'd not expect injuries. Possibly the code that handles adding the % chance and success is working, but something is still adding injuries?

Checking again the missons are actually failing, so I guess it's just adding the % to the displayed chance but not actually affecting the mission at all.
